Wolverhampton Community Stroke Coordinators

This service to support all patients who have had a stroke or TIA was set up in 2008.

Our aims are-

 To assess all stroke patients 2 weeks after discharge from hospital and then provide reviews at 3, 6 and 12 months, all within the patient’s own home.
 To review all patients who have been referred to us from the Rapid Access TIA Clinic at New Cross Hospital within 1 month in our Out-Patient TIA Clinic.

We will address the following-

 How you are recovering since your stroke/TIA

 Whether you are receiving the right services to continue your rehabilitation at home.

 Review risk factors for stroke such as blood pressure, body mass index and pulse

 Advice and information on healthy lifestyle including signposting to other services

 Advice on other issues such as driving, return to work and leisure.
