Intermediate Care Team
The Intermediate Care Team's purpose is to:
• Prevent unnecessary admission to hospital or other care establishments
• Enable early discharge
• Promote recovery at home
• Promote rehabilitation
• Promote independence
We provide a range of hospital home treatments for cellulitus, Respiratory Care, Deep Vein Thrombosis Treatment in the patient’s home.
This service is jointly funded with Wolverhampton City Council.
How to access this service
Patients will be referred to the service by their GP or following discharge from the hospital or any care establishment
Locations where the service is provided
A member of the team will visit the patient at home for up to 6 weeks.

When is the service available?
The Intermediate Care Team work from 8am to 10pm every day.
