Hospital Liaison Service
This service provides support for patients that live in Wolverhampton who have complex needs and are being discharged from hospital. The team provides an assessment and liaison role to help make the discharge process easier for both the patient and their carers/relatives.
The service is provided by hospital based community nursing staff

Access to this service is via referral from ward staff at Royal Wolverhampton Hospital.

The service is available Monday to Friday, 9am – 5pm.

Further Information
Following discharge from hospital, care will be provided by the relevant community nursing team, dependent upon the ongoing treatment and support the patient requires and where the patient lives.
