Heart Failure Team
This service offers diagnosis, treatment and education to patients who have been newly diagnosed with LVSD (left ventricular systolic dysfunction) heart failure
The service offers education sessions for patients with LVSD heart failure which includes advice on exercise, diet, medication and the expert patient programme.
Telephone consultations are also available.

Access to this service id via a referral from the GP, hospital team or other health care professional.

The service can be provided in the home, care home, hospital clinic and other community settings.

When is the service available?
Monday to Friday, 9am to 5pm (excluding bank holidays).
