District Nursing
The District Nursing service provides nursing care for people who are either unable to access this care outside their own home, or if their home is the most appropriate setting.
Patients are assessed and care is planned and evaluated by qualified nurses. Care is given by qualified nurses and health care assistants.
This includes care for terminally ill patients and their extended needs and also patients with any wounds that need nursing care.
The District nursing teams care for many other conditions where a nursing need is identified, and also give information, teaching and support to enable self care of these conditions.
How to access this service
If the patient is currently being seen by the District Nursing service a local contact number will be given.
If a referral is needed the teams are contacted by the GP, social worker or via other health care professionals.

When is the service available?
District nurses work from 8.30am - 5pm and 6pm – 8am, seven days a week
