Diabetes Nursing Service
The service's aim is to increase awareness of diabetes and provide Specialist Diabetes Nursing support for individuals and their families living with diabetes.
The service is provided both within Community Health Centres and the patients own home if needed.
The service attends a large variety of events including city shows and Living With Diabetes Days and works in conjunction with (among others) Diabetes UK, weight management groups, swimming groups, Walking for Health classes and schools. The service also offers education for pregnant women and family members
The service provides:
- Nurse-led clinical assessment and treatment of patients with diabetes
- Advice and support to patients and their families relating to diabetic care
- Lifestyle education and advice on diabetes tailored for patients from minority and ethnic groups and their relatives
- Pregnancy related diabetes treatment
- Telephone advice to patients and healthcare professionals

Patients can access the service via their GP or other Health Care Professional or they can refer directly to the service by calling the number below.
