Community Matrons

Community Matrons are a team of experienced specialist nurses who can work closely with both the patient and their GP to plan and organise individualised health/social care.

Their role is to ensure that patients with long term conditions or a complex range of conditions are offered the opportunity of support from a Community Matron.

Community Matrons aim to prevent unnecessary trips to the GP or to the hospital.

How to access this service
The patient will be identified via the GP who will contact the Community Matron who covers your area.

Locations where the service is provided
Once referred, the patient is able to see their Community Matron at the GP practice. However, if the patient is housebound, the Community Matron will do home visits.

Contact details 
Each patient will be given contact details for the Community Matron who covers their area when they are referred by their GP.

When is the service available?
Community Matrons work from 9am to 5pm, Monday to Friday.

Out of Hours / Emergency Availability
No out of hours service is provided at present.
